  • Perform initial and periodic physical examinations.
  • Evaluate patient behavior and explore patient’s interpretation of health needs.
  • Develop and implement long and short range, treatment plans with the patient and/or family.
  • Furnish and adjust medications according to the law and health center protocols.
  • Develop and maintain problem list and medical record inclusive of physical, psychosocial, environmental, economic, and cultural factors relevant to the patient.
  • Counsel patients and families as to preventative care, medical problems, psychosocial problems, and use of prescribed treatment drugs.
  • Collaborate and consult with other health care providers including medical consultant, regarding disease detection and client management beyond scope of practice.
  • Knowledge of community resources to make appropriate referrals, which will improve quality of life, e.g. nutritional education, physical therapy, substance abuse, smoking cessation, weight loss, psychological therapy, and complementary therapies.
  • Participate in AVH's quality improvement programs, including peer review.
  • Act as a resource to multidisciplinary team in area(s) of expertise.
  • Participate in continuing education programs as needed.
  • Maintain and enhance level of knowledge relative to professional practice and individual areas of interest.
  • After each patient visit is complete, enter accurate and comprehensive progress notes that effectively captures all services rendered including identifying ICD9 and CPT codes.
  • Complete required patient chart notes within 72 hours of every patient visit.
  • Provide an average of 2.6 patient visits each hour in order to accommodate our patient base.
  • Responsible for becoming familiar with the different types of assistance programs available to health center patients.
  • Report births, deaths, and outbreak of contagious diseases to government authorities.
  • Adhere to mandated reporting requirements of child abuse, elder abuse, including suspicion of sexual abuse.
  • Make house and emergency calls to attend to patients unable to visit health center when needed.
  • Educate patients in regards to up to date recommendations for immunizations, according to the Center for Disease Control protocols.
  • Provide after hours on call (telephone triage) five to seven days per month.
  • Proficient in quality patient relations by repeating key information to ensure accuracy, giving information clearly, concisely and calmly, offering assistance, and calling patients or guests by proper name.
  • Proficient in creating a supportive climate by allowing patient and/or family members to verbalize their concerns, providing an interpreter when appropriate and available, and concluding all interactions on a positive and encouraging note.
  • Proficient in creating positive telephone communication with patients and family when appropriate.
  • Interact with patients, physicians, staff, vendors, and visitors in ways that demonstrate caring and reflect the AVH mission, vision, and values.
  • Other duties as assigned.
